Sports Medicine
Welcome to the UDC Intercollegiate Athletics Sports Medicine Office. We, the members of the Sports Medicine Team, wish to welcome you to the 2008-2009 school year. The Sports Medicine Team is a group of allied health providers and physicians who assist the student-athletes with their medical needs. The Sports Medicine Team is currently comprised of certified athletic trainers, general physicians, orthopedics, and cardiologists.
We have posted the following information and forms online to assist you in completing and understanding the requirements to participate in UDC Intercollegiate Athletics.
1. At UDC all student-athletes must complete a yearly physical performed by a licensed physician before they can participate in Intercollegiate Athletics.
· New athletes may use the physician of their choice for the physical exam. You will need to complete the Medical History form and have the Physical Examination form completed and signed by a licensed physician. If you have not received both forms please contact your coach. These forms must be completed and returned before you can begin participation.
· Returning athletes are required to have their physicals performed by the physicians on the UDC Sports Medicine Team.
· New athletes have the option to choose to have their physicals performed by the physicians on the UDC Sports Medicine Team as well. If the new athlete chooses this option they should be aware that this may delay the start of participation due to the possibility of limited physician availability.
2. New student-athletes are required to have a cardiovascular screening and an electrocardiogram (EKG) and be medically cleared to participate by a licensed cardiologist.
· New athletes may use the cardiologist of their choice but will need to provide documentation of the medical clearance by the licensed cardiologist and a copy of the EKG.
· Returning third year athletes are required to have their cardiovascular screening and EKG performed by cardiologists on the UDC Sports Medicine Team.
· New athletes may choose to have their cardiovascular screening and EKG performed by a cardiologist on the UDC Sports Medicine Team as well. If the new athlete chooses this option they should be aware that this may delay the start of participation due to the possibility of limited physician availability.
3. Student-athletes must carry personal accident coverage through a primary insurance policy during the entire school year.
· The University of the District of Columbia provides secondary coverage to supplement the medical accident insurance that the student-athlete must carry.
· Athletes must show proof of coverage prior to participating in any intercollegiate activities.
· If an athlete does not have a primary policy they can be assisted in securing a policy through Health Services or the Athletics Department.
The following six documents are required for participation in Intercollegiate Athletics. If you are not able to print these forms please contact the Head Athletic Trainer and these forms will be sent to you. Please read, complete, sign, and return the following documents.
